Before you start
You need to download and complete an
before you start.We also recommend preparing the following information:
- the licensee’s address and contact details
- the address where the animals are being kept
- county parish holding (CPH) numbers
- designated premises approval number (if you have one)
- animal identification marks such as ear tags, freeze brands, slap marks and tattoos
- stock vehicle and driver details if available
