Sickle cell and thalassaemia high/low prevalence laboratory return

These data templates are for submitting annual screening data from sickle cell and thalassaemia screening laboratories.

There are two templates with different versions for high prevalence areas and low prevalence areas. Use the area prevalence for sickle cell and thalassaemia for NHS trusts guidance to determine the form to use.

Please download and complete either the high prevalence or low prevalence data collection template. Once complete, return to england.screeningdata@nhs.net by 17th July 2026.
