Apply for healthcare cover in the EU, European Economic Area or Switzerland (CA8454)
Use the online service or postal form to apply for a healthcare certificate (this includes a Portable Document S1, E106 or E109E109) in the EU, Norway,European Economic Area, Iceland, Liechtenstein, SwitzerlandNorway or Liechtenstein.Switzerland.
Who can apply
You will need to check if you are entitled to a certificate or document to continue paying National Insurance contributions in the UK. If you are and you reside in the EU, Iceland, Liechtenstein, Norway or Switzerland, then complete form CA8454 to apply for health care cover from the UK for you and your dependants.
HMRC will respond to your application with any action you need to take.
If you’re a student
You may be able to apply for healthcare abroad if you are studying in:
- the EU
- Iceland
- Liechtenstein
- Norway
- Switzerland

What you will need
If you or an agent complete the application you’ll need to say if you:
- have changed your name, if so what your previous name were
- are a national of the EU, Norway, Iceland, Switzerland or Liechtenstein, and if you have an EU Settlement Scheme status
- have more than one nationality ― you’ll need to give details about how you got each one
- have a family member who is a national of the EU, Norway, Iceland, Switzerland or Liechtenstein ― you’ll need their details including their relationship with you
If you’re a mariner
If you’re a mariner you will also need to include with your application a copy of your:
- mariners’ discharge book
- latest payslip
You will need to include the unique reference you get when you submit your form online, on any additional information you send by post.

How to apply
You can apply for a healthcare certificate (this includes a Portable Document S1, E106 or E109E109) in the EU, Norway,European Iceland,Economic SwitzerlandArea, orIceland, Liechtenstein, onlineNorway or by post.Switzerland.
Apply online
To apply online, you need a Government Gateway user ID and password. If you do not have a user ID, you can create one when you apply.
You will get a unique reference when you submit your form online.
To send us further information that cannot be included in your online form you will need to include the unique reference and post it to:

Apply by post
Fill in the form on screen, print it and post it to HMRC.
You’ll need to fill in the form fully before you can print it. You cannot save a partly completed form, so we suggest you gather all your information together before you begin to fill it in.

We have replaced and updated the existing form to apply for a healthcare entitlement certificate. This covers you or any of your dependents if you are taking maternity, paternity or adoption leave in, or for any dependents living in, one of the listed countries.

Change to the application process for applying for a certificate of coverage where you will now also receive a healthcare entitlement certificate if you qualify. Removed mariner section as an applicant does not need to provide this any longer. A new section has been included so that you can check when you can expect a reply from HMRC after you’ve applied.
