Updated the document template and clarified aspects of the text. Requirement change for optional fields to be submitted as -2 if data cannot be provided; only submit 0 for zero certificates. Requirement change to include all qualifications of the pre-populated template in the submission; populate with all 0, instead of dropping rows for qualifications with all zero certificates.
